In January 2023, Andrew Huberman's lab at Stanford published one of the cleanest randomized controlled trials we have on breathwork. Balban, Spiegel, Zeitzer, Huberman, et al., peer-reviewed in Cell Reports Medicine. n=114. Remote, controlled, dose-response across 28 days.
They compared three different five-minute daily breath protocols against five minutes of mindfulness meditation. One protocol won on every primary endpoint they measured: mood improvement, anxiety reduction, respiratory rate, heart rate.
The winning protocol was the exhale.
Specifically: cyclic sighing, a double inhale through the nose, followed by a long, slow exhale through the mouth. The longer-exhale group outperformed box-breathing, outperformed cyclic hyperventilation, and outperformed meditation. Not by a tiny margin. Visibly.
Five minutes a day of exhale-emphasized breath did more for mood and physiological calm than the same dose of meditation.

Why meditation didn't win.
Meditation is not weaker than breathwork. It's slower. Sitting in awareness does eventually shift the nervous system, but it asks the prefrontal cortex to do most of the regulating. Breath asks the body to do it. Body-up is faster than mind-down. About 80% of vagus-nerve fibers are afferent, body to brain, and only 20% efferent. You can't think your way to calm as fast as you can exhale your way there.
